Possibility to add folder art as well as embedded art
Apparently no folder art is added if the art is embedded into the music files. It would be nice if both can be done, if selected.

I finally had a chance to play with this some more and have figured out what is going on (at least in my case). I run bliss on OS X and my music library is stored on a case-sensitive HFS+ volume. In many of my album directories there is uncompliant art named Folder.jpg. Bliss sees this art and correctly marks the album as uncompliant. When I tell bliss to fix this issue, it saves the new art as folder.jpg (as I have told it to do). However, bliss does not remove Folder.jpg. Bliss then will rescan the album in question and recognize Folder.jpg instead of folder.jpg and continue to report the art as uncompliant.

One workaround to try: attempt to change to the same art twice. From the change art screen, choose some art, allow it to re-assess. The album will still be uncompliant. Then choose it again, and see if the folder art takes the second time.
But for fixing it, please, when bliss is running, send me the debug archive from http://localhost:3221/debug (replace localhost with the computer name if bliss is running on a different computer). Also tell me which album this occurred with.

I'm seeing the same thing. I have it set to both embed art and set the image to folder.jpg. In the cases I've noticed, a folder.jpg already exists but is uncompliant. When I tell bliss to fix this and select a new image, it only embeds the new art and does not replace the folder.jpg that is too small. Therefore the album is always uncompliant.

So you are not seeing the file being created in the folder? Is this when art is changed manually? Could you give some detail of the steps you go through?

I have all files of the same album in one respective folder, and would like, when the album art jpg is included in the folder and is also embedded in the music files

A file should be saved in an album folder as well as embedded art, but only if all the tracks in the folder share the same album name tag (or near enough). Are you not seeing that?
